INDIVIDUAL DONATIONS

Your donation will help support CCPC efforts toward the transformation of primary care practice in Connecticut. More specifically, your donation will help CCPC staff create and manage important student internships similar to the House Call Program: A Unique Care Model. These projects are important to the overall training of new primary care providers by exposing the students to real community-based medicine. Students use their critical thinking skills to help resolve issues related to the day-to-day practice of primary care medicine.

In addition, CCPC hosts the Annual Primary Care Summit, a a venue for primary care leaders, clinicians, hospital administrators, managed care representatives, interested public policy stakeholders and advocates, to formulate strategies for the advancement of primary care and appropriate health system reform for the state of Connecticut. Your donation will enable CCPC to continue to give to students in primary care related educational programs free registration to the Annual Primary Care Summit.
