If you are interested in becoming a program sponsor or donating to CCPC, you may fax a completed Sponsor Commitment Form, or send with your check or money order, made out to:

Your company’s corporate sponsorship will help support CCPC efforts toward the transformation of primary care practice in Connecticut. These efforts include studying new models of care, including care coordination, patient safety, and quality improvement initiatives; and translating these new models of care into everyday practice via educational programs for primary care providers and community members throughout Connecticut. Your donation will help CCPC staff create and manage important provider-based research similar to the House Call Program: A Unique Care Model. In addition, CCPC hosts the Annual Primary Care Summit, a a venue for primary care leaders, clinicians, hospital administrators, managed care representatives, interested public policy stakeholders and advocates, to formulate strategies for the advancement of primary care and appropriate health system reform for the state of Connecticut.
